When you schedule an appointment or phone consultation with us, you will have the opportunity to submit your insurance information for verification. We will verify your benefits and contact you with the details of your coverage, usually within 48 hours. Depending on the details of your plan, we will provide complementary billing services for your treatments. If we are not able to bill your plan directly, we will provide you with a superbill receipt so that you can seek reimbursement through your plan. You will be responsible for any portion of your bill that is not covered by your insurance. A fee schedule for services rendered is always available upon request.

For those not using insurance, we accept cash, check or credit card payment at the time of treatment. A “time of service” discount applies to rates for un-insured and under-insured patients who pay at the time of their appointment. It does not apply to patients paying to meet their deductible. This discount acknowledges that there is considerably more time and cost involved with processing insurance claims.
If you miss an appointment or cancel less than 24 hours before your appointment, you will be charged a $50 late cancellation fee. Please understand that we cannot bill insurance for a missed appointment, so this fee will also apply for insurance patients who miss an appointment.
